My report says 53 and my old report says 7.0 — are these two different results?
No — they are the same result in two units. 53 mmol/mol (IFCC, used across most of Europe, the UK and Australia) equals (53 + 23.5) ÷ 10.93 ≈ 7.00 % (NGSP, the US convention). The international consensus formula is IFCC = 10.93 × NGSP − 23.5. Other common landmarks: 6.5 % ≈ 48 mmol/mol (diabetes threshold) and 5.7 % ≈ 39 mmol/mol (lower edge of prediabetes). Many Hong Kong and Singapore labs now print both side by side so clinicians can read either convention.
If my HbA1c is 7.0 %, what is my average blood glucose?
Use the ADAG (Nathan 2008) formula: eAG (mg/dL) = 28.7 × A1C − 46.7. Plug in 7.0 → 28.7 × 7 − 46.7 = 154.2 mg/dL ≈ 8.6 mmol/L. That is your average blood glucose across every minute of the past 8–12 weeks (fasting, post-meal, overnight). Note that eAG is not your home-meter single reading — that one is a snapshot and can swing from 4 to 12 mmol/L; HbA1c is the long-run average of those swings. If your eAG is high but fasting glucose looks normal, post-meal control is often the explanation; a continuous glucose monitor for a couple of weeks can show clinicians where the spikes are.
Why is my HbA1c high even though my home readings look fine — or vice versa?
HbA1c measures how heavily red cells have been glycated, so anything that changes red-cell lifespan distorts it. Conditions that extend RBC lifespan (iron-deficiency anaemia, post-splenectomy, aplastic anaemia) leave more time for glycation and push HbA1c artificially high. Conditions that shorten RBC lifespan (haemolytic anaemia, recent transfusion, pregnancy, chronic renal failure, haemoglobin variants such as HbS / HbC / HbE) push it artificially low, hiding real hyperglycaemia. Your home meter is a snapshot — HbA1c and meter readings are complementary, not competing accuracy claims. When the two disagree badly, clinicians often switch to fructosamine (which reflects the past 2–3 weeks) or fit a 14-day continuous glucose monitor for a finer picture.
