Pack-Year (Smoking History) Calculator

Lung-cancer screening reference

Meets the USPSTF 2021 pack-year threshold (≥ 20 PY). If you are 50–80 years old and currently smoke or quit within the last 15 years, discuss annual low-dose CT screening with a clinician.

Frequently asked

Does smoking even one cigarette a day count toward pack-years?

Yes — proportionally. One cigarette per day is 1/20 of a pack, so a year of that is 0.05 pack-years. The number is small but any smoking still raises cardiovascular and cancer risk.

I have quit smoking — does pack-years still matter?

Yes. Pack-years measure exposure already accumulated and do not reset on quitting. The USPSTF 2021 LDCT screening recommendation covers adults 50–80 with ≥ 20 pack-years who quit within the last 15 years; risk falls over time but stays above never-smokers.

How do e-cigarettes or heated tobacco fit in?

Pack-years were designed for combustible cigarettes; there is no agreed equivalent for e-cigarettes or heated-tobacco products. USPSTF, ACS and other guidelines still base screening on combustible-cigarette pack-years. If you use multiple products, describe usage patterns to a clinician for individual risk assessment.

The Department of Health Smoking Cessation Hotline (1833 183) offers free phone counselling. Public hospitals, MCHCs, staff clinics and NGOs such as COSH also provide cessation services. Pharmacies sell registered nicotine-replacement therapy (NRT) products.
