Body Frame Size (Grant's Wrist Index) Calculator
Pick biological sex and enter height and wrist circumference (measured at the narrowest point just above the styloid). The tool returns Grant's ratio r and a small / medium / large frame classification, used to adjust Devine / Hamwi ideal-body-weight estimates (−10% small, +10% large) and to explain why two people with identical BMI can look quite different.

Grant's index r = height ÷ wrist
11.0
Medium frame
Cut-offs (by biological sex)
Female: small r > 11.0; medium 10.1–11.0; large r < 10.1
Formula
r = height (cm) ÷ wrist circumference (cm) Female: r > 11.0 → small; 10.1 ≤ r ≤ 11.0 → medium; r < 10.1 → large Male: r > 10.4 → small; 9.6 ≤ r ≤ 10.4 → medium; r < 9.6 → large
- · Source: W. C. Grant, "Determining body frame size," subsequently adopted by NIH MedlinePlus and the Healthwise Knowledgebase.
- · How to measure: use a non-stretch tape, snug but not tight, around the narrowest part of the wrist just above the radius / ulna styloid.
- · Wrist circumference is one of the few skeletal indices that is largely independent of body fat, but it can be affected by wrist oedema, prior fractures or arthritis.
- · Frame classification is commonly used to correct Devine (1974) or Hamwi (1964) ideal-body-weight (IBW) figures: −10% small, unchanged medium, +10% large.
- · A single anthropometric ratio has limits; muscle mass, fat distribution and ethnic differences all affect what "ideal weight" actually means. Cross-reference with BMI, waist circumference and body-fat percentage.
Frequently asked
Why not just measure shoulder or hip width directly?
Shoulder and hip widths can describe skeletal frame more precisely, but they need anthropometric calipers and accurate identification of bony landmarks (acromion / iliac crest) — hard to do reliably at home. The wrist is a surface bony point that anyone can find and measure with a soft tape, giving good repeatability with small error, which is why clinicians and dietitians use it as a proxy.
Can body frame size change?
After skeletal maturity, the bones stop growing in length so your frame category is essentially fixed; however the wrist circumference itself can drift a little with oedema, prolonged load-bearing exercise, or endocrine conditions such as acromegaly. A noticeable change over a short period of time is worth flagging to a clinician.
Does a larger frame automatically mean a heavier "ideal" weight?
Yes — people with a larger frame carry more skeletal mass, muscle and connective tissue at the same height, so Devine and Hamwi recommend a roughly 10% upward adjustment to the ideal-body-weight figure. That adjustment is structural, however; it does not change the amount of body fat. Use waist circumference, body-fat percentage and cardio-fitness markers to evaluate actual health risk.
