Why subtract half the sugar alcohols rather than all of them?

Because different polyols absorb at very different rates. Erythritol is essentially 0%, maltitol is 75–80%, sorbitol around 50%. When Atkins coined "net carbs" in the 1990s they used the population average (~50%) as a single rule to keep labels simple. In practice: products sweetened with erythritol (e.g. older Quest bars) hit near-zero net carbs; products sweetened with maltitol spike blood sugar far more than the "half" rule predicts. If a "low net carb" snack still spikes your glucose, maltitol is the most likely culprit — re-run the same numbers in "strict keto" mode for a more conservative estimate.

Why do EU and US labels give different net-carb numbers?

Under EU Regulation 1169/2011 the carbohydrate row already excludes polyols (a separate "of which polyols" sub-row is given), so the headline "Carbohydrate" number is already net of sugar alcohols. The US FDA Nutrition Facts Label is different: "Total Carbohydrate" includes fibre, sugars and sugar alcohols, so you must subtract them yourself. The default mode in this tool assumes a US label; switch to "EU / UK" mode for EU, UK and many Asia-imported products, otherwise you will double-subtract polyols and underestimate net carbs.

For diabetes, should I count net carbs or total carbs?

The ADA 2024 Standards of Care recommend that insulin-using type 1 and type 2 diabetics dose against TOTAL carbohydrates, because sugar alcohol absorption varies (maltitol especially), and the half-subtraction can leave you under-dosed and hyperglycaemic after the meal. For type 2 diabetics managed by diet and oral medication, net carbs (cross-checked with a continuous glucose monitor) is a useful planning tool because it encourages high-fibre foods and zero-calorie sweeteners. Practical rule: use net carbs to choose foods, but dose insulin against total carbs.
